Patient-access frameworks define who can receive medical cannabis and how: qualifying-condition lists or physician-discretion models, specialist versus GP prescribing, patient registries, and reimbursement rules. These design choices determine the real size of a market more directly than any other regulatory factor.
Two structural models dominate: enumerated-condition systems (common in US states and newer programs) and discretion systems where any indication may be prescribed at physician judgment (Germany, Australia SAS-B). Reimbursement is the multiplier — Germany's statutory-insurance coverage decision turned it into the largest medical cannabis market in the EU.
