Pennsylvania enacted the Medical Marijuana Act (Act 16) in April 2016, establishing one of the most expansive medical cannabis programs in the northeastern United States. The Pennsylvania Department of Health oversees the program, which has grown to serve over 439,000 active patients through approximately 185 licensed dispensaries. Cumulative retail sales have surpassed $8.5 billion, making it the sixth-largest cannabis retail market in the country despite being medical-only. The program covers 24 qualifying conditions including anxiety, opioid use disorder, chronic pain, and cancer. Pennsylvania permits up to 25 grower-processors and 50 dispensary permits. The state has seen strong bipartisan interest in adult-use legislation but no measure has passed as of 2026. Home cultivation remains prohibited for patients.
